Randolph Carter begins the story telling the harrowing tale to those who found him stumbling through the swamp. Carter and his friend, the occultist Harley Warren, went to investigate the location of a supposed portal to the underworld - near Big Cypress Swamp in Florida. Upon discovery of a tomb with stairs going deep into the earth, Warren decides to descend them. Only after waiting for what seems an eternity does Carter call down into the tomb, only to be returned with a voice that does not belong to Warren telling him that his friend was dead.

A former intern and a worker of a mental hospital relates his experience with Joe Slater, an inmate who died at the facility a few weeks after being confined as a criminally insane murderer. He describes Slater as a "typical denizen of the Catskill Mountain region, who corresponds exactly with the 'white trash' of the South", for whom "laws and morals are nonexistent" and whose "general mental status is probably below that of any other native American people".

Charles Dexter Ward is a young man from a prominent Rhode Island family who has disappeared from a mental asylum. He had been incarcerated during a prolonged period of insanity, during which he exhibited minor and inexplicable physiological changes. His empty cell is found to be very dusty. The bulk of the story concerns the investigation conducted by the Wards' family doctor, Marinus Bicknell Willett, in an attempt to discover the reason for Ward's madness and physiological changes.

An unnamed narrator pieces together the story of an area known by the locals as the "blasted heath" in the wild hills west of Arkham, Massachusetts. The narrator discovers that many years ago, a meteorite crashed there, poisoning every living being nearby. Vegetation grows large but foul-tasting, animals are driven mad and deformed into grotesque shapes, and the people go insane or die, one by one.

At the Mountains of Madness is a science fiction-horror novella by American author H. P. Lovecraft, written in February/March 1931 and rejected that year by Weird Tales editor Farnsworth Wright on the grounds of its length. This story takes place in the ancient valley of Nis, in vegetation-covered stone ruins described by Lovecraft in great detail. These crumbling blocks of monolithic stone now serve only for grey toads and snakes to nest under. Interspersed in the ruins are large trees that are home to little apes.
