  • She Had Every POTS Symptom. POTS Was Never the Problem.
    Sep 9 2026
What if your POTS symptoms are not simply a “nervous system problem”? In this episode, Dr. Nicole and Dr. Nick take an investigative look at a complex case that raises bigger questions about how POTS, dizziness, fainting, fatigue, blood-clotting issues, autoimmune markers, and vascular compression can be overlooked when care stops at a diagnosis label. Dr. Nicole shares why this case made her repeatedly think, “Oh sh*t, no,” as she followed the clues: a young, high-achieving college graduate with debilitating symptoms, a history that did not add up, an ovary affected by necrosis and torsion, dismissed lab abnormalities, and deeper findings involving hypercoagulability, autoimmune activity, and possible vascular compression. Together, they explore why “normal” standard bloodwork may not always provide enough context, how low-normal or borderline findings can matter when viewed alongside symptoms and history, and why some people diagnosed with POTS may need a more thorough look at circulation, clotting risk, anatomy, immune activity, and upstream physiological stressors. They also discuss the mind-body connection—without reducing physical illness to stress or emotional trauma alone. True root-cause care requires looking at both: the biological drivers affecting the body and the emotional adversity or nervous-system patterns that may be compounding the picture. This is not medical advice or a replacement for individualized care. It is a conversation intended to help you ask better questions, advocate for a more complete evaluation, and move beyond symptom management alone. In This Episode Why a diagnosis can become a stopping point instead of the beginning of an investigationThe limitations of relying only on standard “normal” lab rangesA complex POTS-like case involving ovarian necrosis, clotting concerns, autoimmunity, and vascular findingsWhat hypercoagulability means and why impaired blood flow can contribute to lightheadedness, fainting, and fatigueAntiphospholipid syndrome, autoimmune platelet issues, and positive ANA markersThe possible connection between Nutcracker syndrome, venous compression, varicose dilations, and POTS-like symptomsWhy May-Thurner syndrome and other circulatory or anatomical issues may be worth discussing with a qualified clinicianThe difference between supporting nervous-system regulation and ignoring underlying physiological stressorsWhy healing may require addressing both physical inflammation, toxicity, circulation, and deeper emotional adversity What You Can Do Now Review past lab results with a qualified clinician, especially any results that were labeled “low-normal,” borderline, or positive but dismissed without considering your symptoms and medical history. If you have been diagnosed with POTS or experience persistent fainting, dizziness, pelvic symptoms, leg swelling, unusual pain, or circulation-related concerns, ask whether further vascular evaluation may be appropriate.Discuss whether imaging such as Doppler ultrasound—or referral to an appropriate vascular, cardiology, hematology, or other specialist—could help rule out vascular compression or clotting-related issues.Do not rely on surface-level “nervous system hacks” as the only answer. Consider what ongoing biological, environmental, structural, and emotional stressors may be keeping your system in survival mode. • Why You Wake Up at 3AM in a Panic: Melatonin, Brain Detox and Your Dreams
    Sep 15 2026

Dr. Nicole Rivera and Dr. Nick Carruthers break down why so many people jolt awake between 2 and 3am with worry, fear or full-blown panic — the melatonin-to-cortisol handoff, the brain's overnight detox pathway, and the subconscious material showing up in your dreams.

    What this episode is about

Waking at 2 or 3 in the morning with worry, fear or panic is one of the most common things we hear after fifteen years of practice. Most people reach for the obvious explanations — too much wine, too much caffeine, ate too late. In this episode Dr. Nicole Rivera and Dr. Nick Carruthers walk through the two layers they actually look at.

Layer one — the biochemical story
    • Melatonin does more than make you sleepy. It is also part of how the brain clears what it accumulated during the day.
    • Where the load comes from: heavy metals, dental materials, mouth and sinus products, everyday chemical exposure, and what travels up from the gut.
    • When melatonin gets used up early, cortisol — the hormone that wakes you — arrives hours ahead of schedule. Instead of rising at 5 or 6am, it rises at 2 or 3am, and you wake with a racing mind.
    • Why reaching for a melatonin supplement is a band-aid: it can speed the process up without any of the drainage pathways being open first. Kidneys, liver, lymphatics, neck and sinuses come first.
Layer two — the subconscious story
    • The emotion you wake up in is information. What were you dreaming about?
    • Perceived imbalance — the moments we labelled purely good or purely bad — keeps running at night.
    • Balancing the perception is what settles it. Clarity builds confidence, confidence allows action.
    • Write the fragment down in the morning, even one image, and decode it in the INNSPO app.
