Sam James has spent years carefully crafting her reputation as the best psychologist at Typhlos, Manhattan's most challenging psychiatric institution. She believes if she can't save herself, she'll save someone else. It's this savior complex that serves her well in helping patients battle their inner demons, though it leads Sam down some dark paths and opens her eyes to her own mental turmoil.
When Richard, a mysterious patient no other therapist wants to treat, is admitted to Typhlos, Sam is determined to unlock his secrets and his psyche. But she can't figure out why Richard appears to be so normal in a hospital filled with madness. As Sam gets pulled into Richard's twisted past, she can't help but analyze her own life, and what she discovers terrifies her. And so the mind games begin. But who is the savior and who is the saved?
In this unexpected and addictive psychological debut, A.F. Brady takes readers into the psyche of a deeply disturbed woman desperately trying to keep her head above water, showing that sometimes what's most terrifying is what exists in your mind.

Sam has a cocaine-addicted abusive boyfriend named Lucas, a side male companion who she sees for sex and to feel loved named AJ, and her coworker/best friend named David, whom she had sex with as well. Unbeknownst to Sam, though, David is aware of the others and the abuse and alcoholism Sam tries to hide.
At work, Sam is the best worker and gets great results from her patients, even those that no one else wants to take on. Her boss, Rachel, even confides in her about the company's lack of funding, and she solicits Sam's help because she's overwhelmed with everything going on.
While trying to maintain her status quo at work, hide her tumultuous personal life, and crack the latest difficult patient no one wants to deal with, Sam is starting to spiral out of control. Especially after getting her hands on her latest evaluation. She's forgetting patients, making medication errors, and is the reason one patient finds an opportunity to take his own life.
It's crazy to me that Sam works around so many professionals, yet none of them pick up on the fact that there's more going on with her than exhaustion and migraines. One example is, on the day of her work evaluation, David easily smells the alcohol on Sam and notices that she's hungover, but Rachel, who conducts the evaluation, doesn't smell the alcohol. She just suspects that Sam is tired and suggests she get more rest. There were other blatant incidents as well that caused me to wonder if the other professionals Sam worked with needed to be evaluated, too, because there's just no way her behavior, smell, and look was so often missed.
What an interesting debut novel.
