Good teams get it right about half the time. Great teams work hard, build skill, and get it right most of the time. Exceptional teams excel because they critically review even their best performances to see what wasn't perfect that they could learn from and do better on the next time. It is a process that some call Plan-Do-Review and it was essential to high performance in the nuclear submarine force and for every team I've been on since.
