A skydiver became entangled on a Cessna Caravan's tail after his reserve parachute accidentally deployed at 15,000 feet near Tully, Queensland in September. The experienced jumper, Adrian Ferguson, used a hook knife to cut himself free from the aircraft and successfully deployed his main chute despite remnants of the damaged reserve parachute. The Australian Transport Safety Bureau investigation found the reserve handle snagged on the aircraft's windflap, causing the dangerous situation that damaged the plane's horizontal stabiliser and forced the pilot to make a mayday call before safely landing.
