The American Lung Association states that 70% of smokers want to quit smoking, but need help to do so. More work needs to be done by national, state and local governments to advocate and fund tobacco prevention and control programs, raise public awareness, media campaigns, and ongoing community interventions. The intervention strategies and best practices found herein will equip and empower governments to do so. In 2006 the Hawaii State Legislature passed transformative laws under HRS328J expanding the scope of smoke free prohibitions to include partially and enclosed areas. The prohibition effective January 1, 2016 includes electronic smoking devices (ESDs). Under the Hawaii bill to raise the minimum legal age (MLA) to 21, anyone caught breaking the law would face a $10 fine for the first offense and a $50 fine or community service for a second offense. A 2014 survey of Hawaii voters showed 71% favored raising the legal age to 21, which would result in a 250,000 fewer premature deaths and 50,000 fewer deaths from lung cancer among people born between 2000 and 2019. Prevention requires a concerted societal and political strategy including school-based education, reduced media exposure, counter-marketing, smoke free homes and public areas, and youth access restrictions. Delaying the age of initiation to nicotine significantly prevents a lifetime of addiction considering 99% of lifetime smokers began before the age of 26. This smoking prevention government action plan will therefore preserve public health, save lives, minimize the cost of healthcare expenditures by hundreds of millions of dollars, and empower governments and communities to act intelligently to protect people and the quality of life where they live.
